Which date should I pick?
Their date of birth. If you do not know the time, that is fine: use 12 noon and the map is just as valid, because the stars shift position but they are all still there. If you want the Moon to show its real phase for that day, then it is worth asking for the hour.
And the time?
Without the exact time, noon is the safe convention. Nobody will notice unless they compare two maps of the same day.
